Martin has his second GMC assessment - mock surgery - and Buddy is missing. Meanwhile, Ken's arrival does not bring good news for Al and Bert, and Penhale unhappy with the arrival of his predecessor, Mark Mylow, who tries to take charge of his investigation.

Ruth is concerned about becoming increasingly forgetful, and is even more perturbed when Martin offers to perform a mini mental state examination.

Martin's GMC report requires he passes three refresher courses which will then decide whether he is deemed fit to practice, starting with phlebotomy.

Martin's medical career remains under scrutiny, as a GMC representative has come to assess him and his surgery.

Morwenna’s parents Jack and Tara pay a surprise visit, having been working as missionaries in Kenya. However, the reunion is tinged with sadness when it turns out that Morwenna’s mother has liver cancer, and has decided against treatment because of her faith. Louisa’s first online call for her new course does not go as planned – especially when Martin tells the lecturer exactly what he thinks – and Joe investigates a break-in at the school where nothing has been stolen. Martin Clunes stars in the comedy drama, with Jessica Ransom, Caroline Catz, John Marquez, Kevin Doyle and Rebecca Lacey.

Martin is left holding the baby when Louisa has to take Year Six pupils sailing. He has to collect James from nursery every afternoon, but with surgery hours Martin is constantly late.

Martin and Louisa search for a nursery now that Janice has left, and Mrs Tishell is shocked to find out that Al has very high blood pressure. Louisa also helps with the diagnosis of a pupil, which spurs her on to explore the possibilities of a change of career. This is not welcome news for Martin, who dislikes any kind of change, even if it could be for the better. Elsewhere, Ruth is approached about selling her farm.

Wedding bells are set to ring for PC Penhale and Janice, but nothing ever goes smoothly in Portwenn.

Louisa and Martin have their first therapy session together - though neither of them was expecting they would be given homework - while Morwenna is not happy about it when a teenager on work experience starts trying to boss her around in the surgery. Mrs Tishell is surprised when her husband Clive returns and tells her he wants to give things another go, and Louisa has to call Martin when one of her pupils collapses during a school excursion.

The irascible medic decides to move out of the Surgery to give Louisa space, leaving Mrs Tishell concerned he will not be able to look after himself on his own. Elsewhere, PC Penhale helps Janice babysit, and Al is very concerned when he finds out about Bert's behaviour and hears there are bailiffs at the restaurant.

Al welcomes his first fishing guests, but an uninvited one means that a good review is definitely not guaranteed, while Bert decides to try to rejuvenate trade at the restaurant by joining an online voucher scheme. Morwenna asks Martin for a pay rise, and Louisa has to find a new babysitter to look after James Henry and reluctantly decides to offer Janice a trial. Penhale is excited to receive his new police issue Taser, but Portwenn does not exactly offer him plenty of opportunities to try it out.

With Louisa visiting her mother in Spain to get some perspective on their marriage, Martin knows he needs to try to change for the sake of their relationship, so he endeavours to keep his promise to her to see a therapist. Aunt Ruth arranges an appointment for him with Dr Rachel Timoney, but a lifeboat training exercise causes problems for the doctor. Will he get back in time for his first session?

When Martin is talked into taking baby James to playgroup, he is forced to sing along with the other mums. As the parents pair up, one of them says she is worried about her daughter, to which Martin replies that the girl is probably just backward. Al helps Ruth move into a cottage in the village where she meets her new neighbour, a grumpy old man called Moysey. When Moysey goes for a repeat prescription for his asthma, Martin notices that his breathing has got worse - but he takes exception to being poked and prodded. Al admits he would like a girlfriend and asks Morwenna if she has any single friends. Morwenna asks her mate Janice, who is not impressed by the idea. So a desperate Al decides to sign up with Cornish Couples, a dating website.

A stranger stops dead in his tracks when he sees Ruth in the village, and later he spies on her when she is having a coffee with Caroline. Ruth is not sure why, but she starts feeling uneasy. One of Louisa's pupils blames an upset stomach on a meal she ate at Bert's restaurant and writes about it in the school magazine. Bert and Al are furious, and they go to see Louisa wanting something to be done.

The GP is left holding the baby after upsetting the new childminder, but luckily Mike agrees to look after James at short notice. However, Louisa is not happy with the arrangement and an argument erupts between the newlyweds just as their guests arrive for dinner - and that's only the first of the evening's problems.

Martin and Louisa are about to get married but will it ever happen. Martin is feeling irritated by PC Penhale, who wrongly believes that he is going to be the best man and Louisa is also late for the service. The vicar has serious doubts as to whether she will turn up, leaving Martin on edge. After she finally arrives the couple are married. Things after the wedding don't go as Martin hoped and Ruth has a difficult time looking after James.

Louisa's hippie mother Eleanor arrives in the village, immediately taking over Martin's kitchen to prepare a herbal tonic, which she later gives to the baby to stop his crying. A culinary rivalry develops between Bert and Mark Bridge, the landlord of the Crab and Lobster pub, at Portwenn Fun Day with both keen to take advantage of the event by providing food for the spectators.

New receptionist Morwenna Newcross gets off to a bad start after arriving late on her first day, and her hyperactive behaviour leads Martin to believe she is high on drugs. Ruth tries to deal with an intruder on the farm who has stolen the hub caps from her car, and the sleepless nights begin to take their toll on Martin and Louisa, who are running out of ways to comfort their constantly wailing baby.

The GP returns to his former post at the surgery after the departure of Dr Dibbs, agreeing to stay until a replacement is found. He persuades a reluctant Louisa to move into his house so he can help raise their son, and suggests they could all move to London together in the future. Martin then delivers the eulogy at his aunt Joan's funeral and speaks to her sister Ruth, who proves to be just as cantankerous as he is.

The curmudgeonly GP struggles to come to terms with his new role as a father, having planned to resume his career as a consultant in London before the arrival of his and Louisa's as-yet unnamed son. To make matters worse, he begins to have serious doubts about the competency of his replacement Dr Di Dibbs when she misdiagnoses a patient as well as herself. Martin is then dealt a devastating personal blow after hearing his aunt Joan has died. (This Episode Is Rated TV-PG-V)

A violent earthquake sows panic at the Ambrosiano Hospital. Lin is afraid for her family; Ric decides on Laura's proposal; Federico has to perform a surgery for which he does not feel ready; Martina has an encounter that threatens to bring up her secret; Giulia, on the other hand, has to manage the ward because Doc is stuck in an elevator and has to save a pregnant patient.

Agnese's words have created a rift with Andrea, who must also come to terms with being away from Giulia, who is in Rome for research. Meanwhile, Teresa must work wonders to find a place for Diana's daughter, admitted despite her mother's doubts. Elisabetta is also admitted to the ward after a few days of illness, maybe an indication of a more serious problem.

Doc enters a competition with an AI: if he does not want to be replaced by a machine, he must find the correct diagnosis for a patient before the computer. Martina, on the other hand, is given a case that forces her to come to terms with her conscience, while Agnese is confronted with a terrible decision to protect one of Andrea's secrets.

The fear of discovering the secret about his past is too strong for Doc. However, this denial of the truth drives him away from the ward, much to the surprise of Giulia, who has to take care of a patient who, just like Andrea, seems to be hiding something. Meanwhile, Enrico must deal with Barbara's assaults, which may be hiding more than just infatuation.

Doc finally knows how to track down the woman he is looking for, but he must first find the courage to do so. Meanwhile, Ric faces the case of an undisciplined personal trainer, while Damiano must deal with the hospitalization of a model, his brother Samuele's girlfriend. This case threatens to bring up old family grudges.

The therapy sessions with Enrico seem to have side effects on Doc, who sees his ability to take care of patients called into question. Agnese seizes the opportunity to crush his research into the past, while Giulia is convinced that Andrea should not give up, despite his problems: in fact, his brother Fabio is admitted to the ward.

Doc sees some of the certainties he had about his past collapsing and, to shed light on what happened, he also drags Giulia into his search, who is challenged by the idea that her relationship with Andrea was based on a lie. Angelo, a patient whose condition forces Martina to prove herself, has problems with his past, too.

After a new memory, Andrea finds it hard to believe what his memory seems to be suggesting to him, and he asks Enrico to discontinue treatment. Even on the ward, the situation is far from quiet. In addition to Rita, a young girl who refuses chemotherapy, Andrea and his team have to deal with the case of a patient they know all too well.

The search for Doc's memory seems to have come to a dead end, but Andrea has found a new "sail": the upcoming choir competition in honor of Mattia. While he tries to convince Agnese to accompany him, in the ward Federico ends up under pressure, and Riccardo is faced with a case that sends him into crisis and pushes his relationship with Martina to the limit.

Doc continues to dig into his memory and, now that he has finally met his "sail," he has a clear lead to follow. He has Giulia at his side who, in addition to Andrea's problems, must also think about her career. Meanwhile, a famous travel blogger is admitted to the ward and entrusted to Federico's care. But this encounter is likely to create more than a few problems.

The news of Doc's memory upsets the ward's balance: Agnese fears that recovering his memory will destabilize Andrea's new life, while Damiano is worried about the effect this might have on Giulia. Doc, on the other hand, is increasingly elated, although it is not easy to wedge the therapy prescribed for him by Enrico between the thousands of commitments as head physician.

Again with his white coat, Doc faces his first day as head physician. He has new responsibilities to manage without neglecting patients, despite pressure from the Administrative Director who threatens to close the department. Fortunately, Doc can count on his team, although Julia has not set aside the idea of leaving and Richard is challenged by the new residents.

It's the day of the primary simulation for Doc: Andrea for a day returns to wear the gown, but a sudden computer blackout of the hospital's systems forces him to a series of risky choices.

Agnese is surprised to find Manuel's mother hospitalized, the child she has in foster care with Davide, and with whom things do not seem to go too well. Agnese will have to question everything she thinks she knows, just when Andrea seems to want to be closer to her than ever.

While our doctors are forced to take care of a patient, the rift between Doc and Carolina widens more and more, and Andrea must deal with himself.

To treat an imprisoned mother, accused of trying to kill her daughter, Doc must fight against the prejudices of the entire hospital, just as he himself is being targeted by the investigation.

All the secrets come to light in this special episode, a tribute to the medical heroes of the pandemic.

While a small war breaks out between Doc and the surgeon Valenti because of Carolina, Doc must face the case of a boy with very present parents and is forced to question himself as a father.

Engaged in following two cases at the same time to prove that he is up to the primary, Doc is in direct competition with the new primary and takes several risks.

Dealing with the case of a young tennis phenomenon, Andrea is forced to wonder if it is worth stopping being "Doc" to pursue the primary.

In this special episode, while the investigation conducted by Caruso comes to life and the secrets of the pandemic begin to emerge, Andrea faces a difficult choice: to get back the primary he must take tests, but if he fails, he will never be able to be a doctor again.

While Andrea seems intent on leaving everything, refusing to deal with the difficult case of the nurse Giacomo, a new internist, Damiano Cesconi, and a new infectious disease specialist, Cecilia Tedeschi, arrives in the hospital, but above all a new primary, who seems ready to dismantle the department from the inside.

A train accident turns the Internal Medicine ward into an improvised emergency room. All the doctors are required to do their best to deal with the emergency. One of the wounded is a girl who reopens an important chapter in Riccardo's past that the young man had considered closed forever.

After finding the medical file on the patient who died because of a mistake, Andrea tries to reconstruct what happened before the shooting that wiped out his memory. Marco, who was responsible for this death, and Agnese are uncooperative. Alba and Riccardo struggle to rebuild their relationship and things also get complicated between Gabriel and Elisa.

A patient sheds light on a secret about Lorenzo's private life that causes a stir throughout the ward. Meanwhile, Gabriel and Elisa must attempt to reconstruct the life of an elderly inpatient who appears not to remember anything about himself, but repeatedly utters a single word.

Andrea is confident that he has finally won over Agnese and, feeling romantic, he suggests that a wedding is organised in the ward for a patient in critical condition. Meanwhile, Gabriel makes an unexpected proposal that upsets the balance within the group of interns.

In an attempt to return to his role as a father and to help Carolina recover from her illness, Andrea starts to pursue Agnese again. A bad headache creates problems for Gabriel while treating a patient. Elisa and Riccardo have to deal with an aspiring astronaut.

Carolina, who is suffering from a mysterious illness, is admitted to the ward, triggering a series of conflicts between Andrea and Agnese that they will have to overcome for their daughter's sake. Meanwhile, a flashy influencer attracts the young trainees' attention and Alba cannot disguise her jealousy towards Riccardo.

A special episode set ten years earlier. Andrea, an enthusiastic doctor who is still together with his family, has an opportunity to make a career jump for the first time. But it is a struggle to hold everything together: suddenly finding himself involved in a rivalry with his colleague Marco, he risks losing sight of who he really is.

Giulia brings Andrea back to the ward for a patient who has asked to be exclusively examined by him and who does not know about his current condition. An error caused by his amnesia puts all the trainees under pressure. Meanwhile, Alba is forced to confront the resentment she feels towards her mother.

The arrival of a patient who is particularly important to Giulia intensifies the competition between the interns and Andrea has an opportunity to really get to know them. Meanwhile, he involves the ward in the search for his email account password, which could help him reconstruct a key part of his past, but which someone wants to hide from him at all costs.

Andrea returns to the ward as an assistant to the trainees, who were terrified of him just a few weeks earlier and find it hard to get used to the unexpected role reversal. While he tries to win back his ex-wife, his collaboration with Giulia on the case of a young manager proves to be challenging and full of surprises.

Andrea is hospitalized in the Internal Medicine ward, in the hope that over time the place most familiar to him will help him recover his memory. Giulia oversees his rehabilitation, but finds herself dealing with a different man to the one she knew.

The tranquility of Portwenn is disturbed when a group of birdwatchers descends on the village in search of a rare bird. Pauline announces that she wants to go to university, so Martin has to start looking for another receptionist. Louisa's father Terry arrives unannounced in Portwenn - could this have an effect on the relationship between Martin and Louisa?

Doctor Bamford has had enough of village life and is desperate for some distance from inquisitive Cornish neighbours. When the local estate agent shows him Tregunnt Farm " derelict and miles from anywhere " it's love at first sight. But the Bowden family from London also have their eye on the property with a view to developing the surrounding land into ugly holiday cottages. After being gazumped, the Doc decides to try and spook the family into leaving by pretending to be the Beast of Bodmin.

When obstetrician Martin Bamford learns that his wife has been unfaithful to him with all three of his best mates, he decides to leave London for a short while to clear his head and decide what to do next. He heads to the village of Port Isaac in Cornwall where he had spent some time as a boy. He's a bit of an oddity in the village and some at least suspect he may be there for no good reason. He soon makes friends and finds himself working on a fishing boat and enjoying a somewhat carefree life. Someone in the village is distributing poison pen letters of a sort with messages or photos stuck inside jellies that people find on their doorstep. Soon, some of the villagers point the finger at Martin who thinks those accusing him are daft.
