Milawukee, Wisconsin 4/11/89 – A Sikorsky S-58ET N4247U (November 4247 Uniform) operated by Midwest Helicopters crashes due to a failed coupling on the tail rotor, after carrying an old and faulty air conditioner from a roof. Bristol, VA 4/15/08 – A tanker truck fire is caught on tape by a police dashboard camera.
